An affidavit of service is your sworn statement of how, when, where, and on whom you served a court document. It proves to the court that the other party received the document. You must be at least 18. Anyone can serve, including the plaintiff, an agent, or a process server, except where a rule requires a specific server.

Use the names and claim number exactly as they appear on the issued Plaintiff's Claim, Form 7A. If there is more than one plaintiff or defendant, list them on the line, separated by commas.

Name the exact document by its form number where you can. If you served more than one document at the same time, list them all here. If you served a corporation, name the corporation and, in the capacity field, note who you handed it to.

Be precise about date, time, and place. If service was by mail or courier, the date of service is the date you sent it, and the document is generally deemed served five days later.
